For about $1/day, an individual can enroll to receive their biannual preventive hygiene appointments at no cost. Additionally, all services not fully covered by the premium will be offered at a co-pay that is far less than the usual customary fees.

- 1 oral cancer screening and 1 emergency eval exam (if necessary) per year.

Periodontal Plan
$799 Per Year

- Includes benefits of the Premium Plan
- PLUS up to four additional periodontal maintenance visits per year.
Bright White Plan
